Client employs Seagull Kennels Canine Academy (trainer/kennel) to train/board the above dog. Training to be as follows:
Trainer/kennel agrees to care for the dog properly. Client agrees to pay for veterinary expenses for dog as deemed necessary by Trainer.
It is agreed by client that Trainer/Kennel will not be held liable for loss or damage resulting from injury, disease, death, running away, theft, training accidents, injury to other persons, damages caused by other pets, or any other loss, injury or damage.
It is further agreed that dog will stay with the Trainer/Kennel during training.
Client requests the following services

ð 2) Crate/Housebreaking $225.00 ($75 w/Obed. Training)
ð 3) Boarding (per calendar day) $ 15.00
ð 4) Indoor boarding (per calendar day) $ 25.00
ð 5) Bath/basic groom $ 25.00 +
ð 6) DHLPP & Bordatella (by our vet) $ 35.00 +
ð 7) Rabies shot (by our vet) $ 25.00 +
ð 8) Worming (under 25 lbs. $20, over 25 lbs. $40)
ð 9) Temperament test $50.00
ð 10) **** Follow-up visit (per half hour) $50.00
ð 11) Miscellaneous $
ð 12) Private lessons, one on one with owner (per hour) $100.00
********Please note that the $50.00 follow-up visit only applies if a trainer visits the customer's home/business!
Clients who are having problems with their dog after training is completed can receive follow-up training free of charge at our regularly scheduled monthly training sessions.
Client agrees to pay the fee of $ per dog for training. Client agrees to pay this fee in cash, cashiers check, or money order, training fees are due at signing of contract.
Client also agrees to pay a boarding fee of $15.00+ per day for boarding if dog cannot be picked up after training is completed. Trainer agrees to give client a 24 hour notification that training has been completed.
Clients may visit their dog or the facility during normal business hours by appointment. However, since this is additional time and not calculated into our training fee, it will be billed to the client at a rate of $50.00 per half hour.
There will be a $50.00 service charge for any deliveries after 1:00 P.M. on Saturday or anytime on Sunday.
